Title:Cervical branch of the facial nerve in leprosy.

Abstract:This study demonstrates that the platysma is occasionally palsied in leprosy and that this only occurs when the facial nerve already has some other palsy. That there needs to be a facial palsy before there can be a platysma palsy is strongly suggested, in that there was no case of an isolated platysma palsy. Patients, regardless of age or other factors, could mimic a platysma contraction. This obviates the need for electrical testing to examine for a platysma palsy. It also means that a nonfunctioning platysma on clinical examination is, in fact, a palsied platysma. While lagophthalmos is regularly examined for, and any obvious facial paresis would be noticed, less severe forms of facial muscle paresis will only be found if formally examined for. The mechanism whereby the facial nerve is involved in leprosy is not clarified, but our findings suggest that proximal spread of a lesion that began in the zygomatico-temporal branches and reaches to the facial nerve trunk is more likely than new lesions developing de novo in other peripheral facial nerve branches. That the primary lesion is within the facial nerve trunk in all cases but we only see the frequent zygomatic sequelae due to secondary factors is not excluded. (AU)^ien.

Abstract:Os AA. consideram a posição da lepra dimorfa ("borderline") na classificação, baseados em 28 casos e nos seguintes elementos: características clínicas, bacterioscopia, histologia, prova lepromínica, freqüência e origem. Julgam que a lepra dimorfa é apenas uma fase de transição, que raramente ocorre no curso da moléstia ou por influência da terapêutica. Em alguns doentes, comporta-se de modo semelhante à lepra tuberculóide em reação, regredindo após alguns meses de tratamento, enquento em outros, exige até 3 ou 4 anos para sua evolução. Como a lepra evolui em "câmara lenta", esta fase persiste por tempo maior ou menor, mas constituiria apenas um fato de passagem entre uma forma e outra. Sendo assim e pela sua raridade, pensam que o grupo dimorfo deve ser exclu¡do da classificação; está, além de ser científica, deve possuir também cunho prático, abrangendo, de preferência, as formas comuns da molèstia e nâo raras ou os aspectos transicionais. Os AA. aguardam a oportunidade de ampliar o material e, inclusive, obter ou apreciar os resultados de exames sorológicos e bioqu¡micos, para confirmar ou modificar a presente opinião. (AU)^ipt.

Abstract:A brief review of the contradictory opinions of different authors as to the nature and the place of tuberculoid cases of leprosy in the evolution and pathogenesis of the disease is presented. Mention is also made of their failure to agree on what could be generally considered as the esential histological features of this condition. A case of tuberculoid leprosy which has undergone transformation into a lepromatous-type case within six years of observation is presented. This case report together with the clinical, bacteriological and histological studies of other authors seem to agree with the opinion of Manalang that tuberculoid leprosy is a stage in the evolution of leprosy, not immutable as assert by Schujman. That such transformation among adult tuberculoid cases is rare because they represent the filtered resistant ones or benign infections, is indicated. The need for continued follow-up and life-long study of contact children born of leper parents in institutions is indispensable if we are to know the evolution of leprosy. (AU).

Abstract:Numerous clinical and serological similarities exist between lepromatous and dimorphous leprosy and diseases od collagen, particularly lupus erythematosus and rheumatoid arthritis. The clinical manifestations include spontaneous skin-ulcerations, ischaemic necrosis, petechial and purpuric eruptions, vesicle and bulla formation, subcutaneous nodules, hepatosplenomegaly, migratory arthralgia and polyarthritis, bizarre skin lesions including butterfly facial rashes, and a general tendency toward exacerbations and remissions. Haematological findings include anaemia and the ocasional appearance of lupus cells.Positive serological findings include rheumatoid factors, circulating thyroglobulin antibodies, false positive serological tests for syphilis, and cold precipitable proteins (a consistent finding). Because of these similarities, the authors believe that leprosy is a disease which should be considered as a model for the study of hyperglobulinaemic states, especially the diseases of collagen. An important consideration is that, unlike these other diseases, the agent causing leprosy is known.

Abstract:Leprosy can occur at all ages, in both sexes and in every race. It can be found in any geographic area and can simulate many diseases, especially those affecting the skin. The lepromatous type indicates less host resistance than the tuberculoid type. Histopathologic examination of a properly stained specimen obtained by skin biopsy is the most important diagnostic procedure. Treatment consist of long-term chemotherapy with sulfone drugs.

Abstract:The question of the prospect for absolute cure, oron the other hand that of transformation to the lepromatous type, of active major tuberculoid cases, especially those that at one time or another have been found bacteriologically positive, is discussed. Out of twelve such cases observed at Cebu for several years, no less than seven have relapsed at one time or another after apparently complete recession of the disease. In one instance there had been a period of five years without trouble. It is suggested that the feature of relapse in tuberculoid leprosy is worthy of more attention than it has received. Lepromatous transformations has not as yet occurred in any of these cases. Two of them, however, have persisted for three years and more in an unstable, borderline condition, the picture dominated by repeated papular eruptions, the lesions atypical clinically and histologically. Another borderline case of decidelly different aspect seen in China is cited; it presented at the same time apparently lepromatous, tuberculoid and intermediate lesions and was looked upon as transitional, yet later recession occurred as is common in ordinary tuberculoid leprosy...AU.

Abstract:Los sintomas de principio de la lepra observados en enfermos menores de 15 años, fueron: Tipo lepromatoso: 1) caida de cejas y pestanas. 2) rinitis obstructiva. 3) eritema purpúrico-necrótico. los nodulos son rarisimos como signo de principio; las manchas o placas infiltradas color sepia tambien son raras. Tipo Tuberculoide: 1) placas eritematosas de bordes bien marcados. 2) Lesiones troficas precoces de las manos. Tipo Indiferenciado: 1) manchas acromicas anestesicas localizadas por orden de frecuencia en las regiones gluteas, lumbar, muslos y espalda. (AU).

Três meses após o início do tratamento, apresentou surto reacional (Reaçäo tipo 1) com acentuaçäo da infiltraçäo e do edema na lesäo inicial e o aparecimento de várias outras placas, também eritematosas, disseminadas por todo o tegumento. Nessa ocasiäo, a dor na perna continuava e näo parecia relacionada com a hanseníase. Pelo fato de eletroneuromiografia ter sido inconclusiva, foi realizada uma ressonância magnética que acusou um neurinoma no nível da 4ª vértebra lombar que a histopatologia mostrou tratar-se de um schwanoma benigno. O tumor foi retirado cirugicamente e a dor desapareceu...(AU).

Abstract:Um paciente com história de no mínimo 12 anos de evoluçao, considerado como portador de Hanseníase virchoviana e com todas as seqüelas características (madarose, queda de pirâmide nasal, fibrose e atrofia dos testículos) além de episódios de Eritema nodoso hansênico, apresenta padroes diversos de reaçao granulomatosa em pele, nervos e vísceras. Observa-se granulomas virchovianos ativos, granulomas virchovianos regressivos, granulomas dimorfos difusos, e granulomas com padrao tuberculóide na presença de rica baciloscopia incluindo bacilos típicos. O aspecto mais interessante é o encontro de granulomas tuberculóides múltiplos em linfonodos, medula óssea, fígado e baço. A rica baciloscopia e a presença de bacilos típicos, apesar do predomínio de reaçao granulomatosa de padrao tuberculóide fala a a favor de uma piora da doença. Discute-se a interpretaçao imunológica deste tipo de reaçao e sua relaçao com a má evoluçao do paciente. (AU).
